feat: configurable host port via HOST_PORT in .env - #29

Use ${HOST_PORT:-3000} for the host side of the port mapping and document
HOST_PORT in .env.example. Container-internal port stays 3000 so the image
healthcheck is unaffected. Defaults to 3000 when unset.

Why
Follow-up to #28. Deployments often need SimHub on a host port other than 3000 (proxy in front, port already taken). This makes the host port configurable without editing
compose.yaml.Changes
